Windows 10 and 11 have a feature called SmartScreen that checks unrecognized apps against a Microsoft database. Even if the file is clean, SmartScreen may silently prevent it from launching, and the calling process (like a batch script or another loader) reports "Failed to start."
If the file was accidentally deleted or moved, the error will appear.
